Course Overview

Many patients live alone, making normal daily activities difficult or impossible. For those in a hospital or long-term care setting, days and nights can be boring or lonely. Changes in health status and/or abilities can lead to depression and feelings of isolation. This module will discuss the importance of social support, how to identify important social support people, and how to coordinate support for the patient’s benefit.

In this module you will:

  • Learn about family roles and their significance to the health of the patient
  • Learn how to identify social support people available to the patient
  • Learn how to develop a plan to assist patient/residents to reach the optimum level of independence
  • Be able to develop a workplan with family, friends, and neighbors
